Immunization Records

In accordance with WAC-246-105-030, the State of Washington mandates that all students must have a medically verified Certificate of Immunization (CIS) or Certificate of Exemption on file before commencing school.

Medically Verified Immunization Records

Here are some examples of medically verified immunization records:

  1. A completed Certificate of Immunization Status (CIS) signed by a healthcare provider. You can find the CIS form here under “Certificate of Immunization Status.”
  2. A CIS filled out by a parent or guardian with medical records attached.
  3. A CIS printed by a healthcare provider or school from the Washington State Immunization Information System.
  4. A CIS printed from MyIR, a free Department of Health online tool that allows families to view and print their official immunization records themselves.

Immunization Requirements

Please note, students enrolling in school from preschool through 12th grade, either for in-person or online learning, must fulfill the necessary immunization requirements as listed in the 2023 - 2024 School Immunization Charts (English) or 2023 - 2024 School Immunization Charts (Español). This also applies to students of the same age range participating in before and after-school programs.

Immunization Exemptions

If you wish to request an exemption from one or more of the immunization requirements, you must provide the school with a completed Certificate of Exemption. To learn more about exemptions, please visit theDepartment of Health website.(

Exception

Students eligible for the McKinney-Vento program are not required to provide health documents at the time of enrollment. If there are barriers for the family to access these documents, please contact the McKinney-Vento liaison.

In preparation for your student's enrollment, please ensure that you have a medically verified immunization record. This can be obtained from your student's healthcare provider or accessed and printed independently using MyIR, a complimentary online tool from the Department of Health.

We value the health and safety of our students and appreciate your cooperation in fulfilling these requirements.

Residency Verification

Proof of residence can be substantiated through various documents, including lease agreements or a recent utility bill. Accepted utility bills include those for gas, water, or electricity. They should feature the parent or guardian's name, address, and a date within the past 30 days. In situations where utilities are incorporated into your rent, a rental agreement on the property management's letterhead can be submitted as verification.

Kindergarten Registration

To enroll in kindergarten, a child must turn five years old by August 31. During pre-registration, we confirm the child's birth date using one of several acceptable documents. These include a state-issued birth certificate, a hospital-issued certificate of live birth, a passport, adoption records, or a parent's affidavit. For more details or inquiries, please consult your child's school office staff. Additional information specific to kindergarten enrollment can be found on our Kindergarten Registration page.

Grade Level

At NKSD. grade level placement is determined by considering several key factors. These include the student's previous academic records, age, and any available testing results. The district aims to place students in the grade level that is the most suitable for their academic and social development, ensuring they are in the best position to succeed. It's important to note that verification of the student's previous schooling and grade level may be required during the enrollment process.

The Free and Reduced Lunch program offers meals at a significantly discounted rate or at no cost to qualifying students. Parents or guardians have the opportunity to apply for this program during the registration process.

If you wish to apply later in the school year, there are two convenient options available to you. You can fill out the "Online Free/Reduced Meal Application" accessible via Skyward Family Access. This digital platform makes applying from home a simple process.
